Understanding sustainability of federal systems change projects is the key to seeing how funding works and the opportunities that funding can present. Using the U.S. Department of Health and Human Services OWH Multidisciplinary Health Care Models for Women, Altarum Institute studied factors that influence program sustainability at model sites. Funded sites included both academic and community health centers in a variety of settings, including rural and urban. The methodology included data abstraction from applications and reports, a Web-based survey, in-depth qualitative interviews, and site visits to 12 centers. The final products included a report discussing implications for federal agencies approaches to sustainability and manuscripts for submission to peer-reviewed journals.
